HEALTH BENEFITS

Blue Care Network (BCN) is the medical and prescription drug plan insurance carrier.  The plan is a HMO with in-network network coverage available.

The company matches employee HSA contributions dollar-for-dollar up to $750 Individual or $1,500 Family annually.

Delta Dental manages our dental plan.  Use a Delta Premier or Delta Preferred network dentist to receive higher benefit coverage and no claim filing or balance bill requirements.

EyeMed administers our vision plan with Unum as the insurance carrier.  When you use an EyeMed Insight network provider, you pay discounted fees with no claim filing required.  Reimbursement for non-network services is also available.
